A clear plan for progression would be put in place so that values and outcomes are met. This would be measurable at every stage of the journey. Families, friends and professionals are welcome to visit the home/service before the assesment takes place to see the homely feel we have created at Oakleigh.

Pre assesment

The manager will come out to see the person wishing to access the home and do a full assesment. This will ensure that the home can meet the individuals needs and abilities and that we can provide positive outcomes.


We like the person using our service to come along and meet the staff. A day visit is recomended then an overnight stay to ensure smooth transition.
